Requirements
  • Graduate from an accredited School of Nursing.
  • Hold current appropriate state licensure and meet practice requirements in the state of employment.
  • Successfully complete a training course in the theory and practice of hemodialysis.
  • Successfully complete cardiopulmonary resuscitation certification.
  • Meet the necessary requirements of the Ishihara Color Blindness test as a condition of employment.
  • Provide coverage at any or all area facilities as required by management.
  • Complete International Classification of Diseases, Ninth Revision coding training.
  • Complete nurses technical training.
  • Meet appropriate state requirements, if any.
  • Be able to perform frequent prolonged standing, bending, heavy lifting and moving of patients, and occasional assisted movement of machines or equipment weighing up to 200 pounds and chemical or water solutions weighing up to 30 pounds.
  • Work in a healthcare environment with possible exposure to infectious and contagious diseases or materials.
Responsibilities
  • Direct Patient Care Technicians in providing safe and effective chronic hemodialysis therapy in compliance with facility policies, corporate standards, and state and federal regulations.
  • Delegate tasks to direct patient care staff, including licensed vocational or practical nurses, Patient Care Technicians, and Dialysis Assistants.
  • Assess daily patient care needs and develop appropriate patient care assignments.
  • Monitor patient care staff for proper techniques and adherence to facility policies and procedures.
  • Participate in staff training and orientation of new staff as assigned.
  • Participate in required staff meetings.
  • Educate patients and families about end-stage renal disease, renal disease, vascular access, dialysis therapy, diet and fluid management, medications, laboratory values, treatment compliance, adequacy measures, transplant options, and treatment modalities.
  • Provide safe and effective care to patients with end-stage renal disease.
  • Implement treatment prescriptions, including sodium modeling and ultrafiltration modeling when appropriate.
  • Assess patient responses to hemodialysis therapy and adjust the treatment plan as indicated by the prescribing physician.
  • Communicate patient issues, problems, and concerns to the Team Leader or physician.
  • Initiate initial and annual nursing assessments and continuously evaluate and document patient care needs.
  • Participate in the pre-evaluation, initiation, monitoring, termination, access homeostasis, and post-evaluation of hemodialysis treatment.
  • Intervene when patient adequacy status changes and troubleshoot access-flow issues identified by Online Clearance Monitoring or the Adequacy Monitoring Program.
  • Provide, supervise when applicable, and monitor hemodialysis access care.
  • Implement, administer, monitor, and document prescribed interdialytic transfusions, including notification of physicians and blood suppliers about adverse reactions.
  • Ensure accurate and complete Patient Care Technician documentation on Hemodialysis Treatment Sheets.
  • Review, transcribe, and enter physician laboratory orders into the Medical Information System.
  • Prepare laboratory requisitions and ensure correct specimen tubes, collection, processing, packaging, and delivery procedures.
  • Follow up on missed or insufficient laboratory specimens.
  • Report alert, panic, and abnormal laboratory results to the appropriate physician and forward laboratory results as requested.
  • Enforce company policies, procedures, and applicable state and federal regulations.
  • Maintain safe, efficient, and effective shift operations.
  • Act as a resource for other staff members.
  • Meet with the Clinical Manager regarding personnel and patient-care status, issues, and information.
  • Collaborate with physicians and other healthcare-team members to interpret, adjust, and coordinate patient care.
  • Assist patients with prescription refills according to company policies.
  • Maintain the facility drug list for required stock medications.
  • Maintain competency in emergency procedures and initiate cardiopulmonary resuscitation and emergency measures during cardiac or respiratory arrest.
  • Verify the availability of adequate emergency equipment.
  • Ensure appropriate vaccinations, immunizations, and annual tuberculosis testing.
  • Administer prescribed medications and document medical justification when indicated.
  • Administer as-needed medications and document assessments of effectiveness.
  • Maintain controlled-substance records as required by law.
  • Assist with coordinating patient transportation when necessary.
  • Maintain a clean, safe, and sanitary dialysis treatment environment.
  • Ensure competency in the safe and effective operation of dialysis-related equipment.
  • Ensure patient stations, machines, and chairs are clean and properly positioned.
  • Address blood spills immediately according to bloodborne-pathogen control policies.
  • Enter physician orders, laboratory results, vital signs, treatment parameters, patient status, and other treatment data into the Medical Information System.
  • Implement patient admission, transfer, and discharge procedures.
  • Verify the accuracy of Patient Care Technician documentation and daily completion of Hemodialysis Treatment Sheets.
  • Follow opening and closing procedures and monitor that staff and patients have safely left the premises.
  • Initiate, document, and complete ongoing Continuous Quality Improvement activities, including monthly reports.
  • Complete monthly nurses' progress notes.
  • Ensure patient medical records contain complete and appropriately identified documentation.
  • Review transplant status and follow established procedures.
  • Complete patient care plans for new patients within 30 days and for unstable patients requiring monthly plans.
  • Complete due long-term programs.
  • Complete initial and annual Nursing History and Assessment physicals.
  • Ensure completion of the Annual Standing Order Review with each physician.
  • Perform additional duties as assigned.

  • At least one year of medical-surgical nursing experience is preferred.
  • Hemodialysis experience is preferred.
  • Intensive care unit experience is preferred.
