Requirements
  • A bachelor's degree in Social Work, Psychology, Education, Criminal Justice, or a related field is required.
  • Two years of experience in a behavioral health or community service setting are required.
  • Strong organizational skills are required to manage multiple priorities and meet deadlines.
  • Excellent communication and interpersonal skills are required to work independently and as part of a multidisciplinary team.
  • Demonstrated cultural competence and the ability to work with diverse populations are required.
  • A valid Pennsylvania driver's license and access to a reliable vehicle are required.
  • Current auto insurance meeting minimum collision coverage requirements is required.
  • Act 33, Act 34, and FBI clearances are required, including institutional clearances when working with incarcerated individuals.
  • The ability to complete required onboarding training within 30 days of hire is required.
Responsibilities
  • Serve as the Single Point of Accountability for clients and families, acting as the primary contact and advocate for services.
  • Build and maintain relationships with clients through outreach and engagement.
  • Develop comprehensive service plans tailored to individual strengths and needs, and update them as necessary.
  • Coordinate care across behavioral health, drug and alcohol services, educational supports, and community resources.
  • Maintain monthly face-to-face contact with clients and ensure services are provided in line with program standards.
  • Work with families, probation officers, educational staff, and community providers to address non-academic barriers to learning and promote recovery.
  • Facilitate interdisciplinary team meetings and court service plans, provide follow-up, and ensure continuity of care.
  • Advocate for clients by helping remove systemic barriers, resolving issues, and fostering full inclusion within their community.
  • Complete assessments, service plans, and case notes within specified timeframes in compliance with state and agency regulations.
  • Maintain accurate electronic health records of interactions, family meetings, service linkages, and follow-up activities.
  • Comply with HIPAA confidentiality policies and participate in regular audits and program evaluations.
  • Participate in ongoing training and development opportunities and stay updated on best practices and trends in mental health, juvenile justice, and service coordination.
  • Attend school-based meetings and provide educational advocacy as needed.
