About the position:
Stellar Health is the industry leader in provider performance and changing behavior. The company is now embarking on a new journey into total cost of care management. To continue our mission to empower the provider, Stellar is offering care planning/navigation services for its ACO members. The Patient Care Lead will develop individual care plans for identified patients who have complex healthcare needs. Working in collaboration with internal and external resources, the Patient Care Lead will help their patient: 1) self manage their health 2) identify and remove barriers to proactive healthcare services and 3) establish and achieve self directed care goals. Reporting directly to the Chief Medical Officer, this individual will be critical to the Stellar ACO success.
As a Patient Care Lead, you will:
Help patient self manage their care
- Provide necessary information to understand and prevent diseases and to help patients manage health conditions (including chronic disease)
- Use motivational interviewing to assess patient needs and develop self care goals
- Provide health promotion and self-management training to increase patients’ self advocacy capabilities
- Create ongoing processes for patients and family/caregiver(s) to determine and request the level of care coordination support they desire at any given time
Identify and remove barriers to proactive healthcare services
- Educate patients and family/caregiver(s) about relevant community resources
- Serve as point of contact and facilitate meetings between members, family/caregiver(s), care team, payers, and community resources
- Increase continuity of care by managing relationships with primary care providers, ancillary services and community resources
- Follow up on health and social service encounters with individuals and families, in coordination with primary care providers and facility staff
- Advocate on behalf of patients with health care professionals
- Coordinate with hospital staff on discharge plans as necessary
- Coordinate transportation to services
Establish and achieve self directed care goals
- Create and promote adherence to a care plan, developed in coordination with the member, primary care provider, and family/caregiver(s)
- Cultivate and support primary care and specialty provider co-management with timely communication, inquiry, follow-up, and integration of information into the care plan regarding transitions-in-care and referrals
- Promote timely access to appropriate care and increase utilization of preventative care
- Monitor adherence to care plans, evaluate effectiveness, monitor member progress on time, and facilitate changes as needed
- Document and track individual and population level data
As a Patient Care Lead, you should have:
- Medical literacy and a minimum of 3 years experience in a medical setting (ie medical practice, healthcare provider, healthcare insurance company, etc)
- At least 2 years of experience doing healthcare planning for patients with more complex healthcare needs
- Effective computer skills, particularly Microsoft Office and G-Suite
- Familiarity with Population Health tools and navigating electronic medical records
- Bachelors degree required
- Excellent communication skills
- Demonstrated ability to motivate others and instill positive perspective
- Resourceful individual who can often solve problems independently
- Empathetic disposition
Pay:
At Stellar, we believe in transparency and we do our best to make sure the company and our candidates are on the same page as it relates to compensation. In addition to posting salary ranges for our open roles, candidates should expect to be asked about compensation expectations and requirements early on in their interview process. Our goal is to highlight when expectations and Stellar’s salary range may be out of sync, and work with the candidate to determine whether it makes sense to continue conversations.
Where a new hire falls within this range will be based on their individual skills and experience, and how these competencies compare across other employees in the same role. Stellar’s bands are designed to allow for individual compensation growth within the role. As such, new hires typically start at the lower end of the range. Stellar rewards performance and outcomes - should you join the company, you will have the opportunity to grow your salary over time.
- The base salary range for the Patient Care Lead is $100,000-$120,000 and will be eligible for equity and an annual performance based bonus.
