Requirements
  • The candidate must hold an active, unrestricted Nurse Practitioner license.
  • The candidate must hold board certification through AANPCB or ANCC in family, adult, geriatric, or adult-gerontology practice.
  • The candidate must hold an active DEA license and controlled substance registration, as required.
  • The candidate must have at least 3 years of clinical experience as a Nurse Practitioner.
  • The candidate must hold Basic Life Support certification.
  • The candidate must have a valid driver’s license and reliable transportation.
  • The candidate must have experience with electronic health record documentation; athenahealth experience is preferred.
  • The candidate must demonstrate a commitment to equitable, patient-centered care.
Responsibilities
  • Provide ongoing primary and preventive care to older adult patient populations.
  • Manage and grow a dedicated patient panel within assigned communities.
  • Deliver relationship-driven, patient-centered care in community settings.
  • Collaborate with interdisciplinary clinical and operational teams.
  • Support initiatives in preventive care, chronic disease management, and care coordination.
  • Use telehealth and in-person care delivery models to improve accessibility.
  • Maintain accurate, timely electronic health record documentation and ensure coding specificity.
Desired Qualifications
  • A background in family medicine, internal medicine, adult medicine, or geriatrics is strongly preferred.
  • Experience in value-based care settings is strongly preferred.
  • Experience providing care in community-based, in-home, or non-traditional healthcare environments is strongly preferred.
  • Familiarity with MEAT documentation and chronic condition coding is strongly preferred.
  • Telehealth experience is strongly preferred.
