- The candidate must hold a current Texas Licensed Vocational Nurse license.

- Provide comprehensive care in patients' homes, including completing skilled documentation through Homecare Homebase.
- Serve as the patient's advocate and maintain ongoing communication with physicians, referral sources, caregivers, and facility management to coordinate care.
- Administer medications and treatments as prescribed by a physician.
- Provide direct patient care, including infusion therapy, wound care, teaching, and disease management.
- Participate in regular case conferences and in-service training.
- Provide service to approximately six patients per day, Monday through Friday.
- Complete at least 30 productivity points per week, with the opportunity to perform additional points.
- Previous home health experience is preferred.
