Requirements
  • A Master’s degree in nursing and graduation from an accredited Nurse Practitioner program are required.
  • One to two years of clinical experience as a Nurse Practitioner is required.
  • At least one year of experience in a geriatric specialty is required.
  • A New York State Registered Nurse/Nurse Practitioner license, registration, and Drug Enforcement Administration certificates are required.
Responsibilities
  • Provide and/or coordinate primary clinical care services for assigned nursing home residents.
  • Develop care plans and ensure access to preventive care services in accordance with member and plan health outcome goals and objectives.
  • Act as a team member and clinical resource to participating providers and facilities.
  • Act as a resource and contact for members or their designated representatives regarding all aspects of the Advantage member care plan.
  • Provide care to members residing in assisted living and long-term care facilities and to members living at home, in collaboration with a physician.
Desired Qualifications
  • Previous clinical Registered Nurse experience in acute, subacute, and/or long-term care settings is preferred.
  • Prior managed-care experience is a plus.
