Requirements
  • Bachelor’s degree in vocational rehabilitation, health administration, social work, sociology, psychology, or education or the equivalent in a related field
  • Minimum of 5 years of successful experience working in social services, mental health and/or substance use, or a related area
  • Experience implementing partnerships and experience in the implementation of community mental health service models preferred
  • Ability to work with people from diverse backgrounds and be sensitive to maintaining confidentiality
  • Highly organized and detail oriented
  • Proficient in Microsoft Office
  • Experience in working with public assistance recipients, formerly incarcerated individuals and those affected by poverty
Responsibilities
  • Daily intake of new members for enrollment into the program
  • Develop a C2C plan for implementing the required interventions with technical assistance
  • Provide care coordination to Jobs-Plus members with clinical referrals including warm hand-offs and follow-up
  • Daily use of validated tools to screen consenting members for mental health symptoms and re-screen at appropriate intervals
  • Daily documentation of participant services and activities in management information systems
  • Bi-weekly engagement with Housing and Resources Authority and partners using qualitative and quantitative data to identify mental health needs and service use patterns
  • Develop policies and procedures for delivery of Jobs-Plus services that are trauma-informed and based on best practices
  • Coordinate quarterly initial training for and/or provide training to staff in the selected interventions and the C2C model
  • Implement interventions according to the HRA-approved plan
  • For psychoeducation, implement at least two selected strategies per year
  • Provide continuing coaching and supervision to Jobs-Plus staff implementing the interventions
  • Make off-site referrals for clinical care ensuring access within 30 minutes travel and within wait times
  • Continue to adapt and expand interventions with HRA approval
  • Monthly case conference with other staff to address individual participant goals and needs
Desired Qualifications
  • Experience in coordinating staff trainings and continuing coaching and supervision, delivering coaching directly as needed
  • Mental Health First Aid (MHFA) training preferred
  • Motivational Interviewing (MI) training preferred
  • Psychoeducation strategies implemented through various modes including one-on-one, group sessions, stand-alone conversations, or sequential conversations
  • Screening tools for mental health symptoms and referral protocols
  • C2C plan details including staff training assignments and referral protocols
