Requirements
  • Nursing diploma.
  • Previous clinical experience with wound care, case management, or patient education.
  • Experience facilitating the design, implementation, and evaluation of health care promotion programs and or community development.

Responsibilities
  • Demonstrates ability to work cooperatively within the framework of the interdisciplinary team. Participates in and provides on-going communication with other team members including, but not limited to team conferences, supervision meetings, and direct patient/family interactions.
  • Consult with team members in development of referrals and Resource recommendations.
  • Maintains on-going communication with multi-disciplinary team members regarding patients' status and follow-up.
  • Documents consultation, recommendations, and interventions.
  • Develop strong community outreach connections and referrals.
  • Formulate relationships with members of the community to increase wound care health awareness and involvement in community programs.
  • Coordinate or conduct limb preservation health education classes with individuals and groups.
  • Identify target population by utilizing community service data.
  • Implements plan of care and strategies to meet the anticipated clinical, quality, and financial outcomes.
  • Refers to internal resources of the hospital that can assist patients and families.
  • Refers to basic community resources.
  • Provides referrals to patients/families for ongoing assistance.
  • Encourages patients and family participation in the decision-making process.
  • Protects patient privacy by maintaining confidentiality.
  • Track patient referrals and quality outcomes.
  • Develop a database to track patients, interventions and their outcomes.
  • Utilize data in the evaluation and improvements of the Limb Preservation Program.
  • Report program progress and effectiveness on a monthly and annual basis.

  • Bachelor's Degree in Nursing
  • Master's Degree in Nursing
  • Experience with patients with diabetes, wound care, or case management.

Sharp Rees-Stealy opens 75,000 sq ft medical centre in Chula Vista with 375-space car park

PMB, a healthcare real estate developer, has completed a 75,000-square-foot medical outpatient building for Sharp Rees-Stealy in Chula Vista, California. The three-storey facility consolidates services from two existing sites, offering expanded primary, urgent, specialty and rehabilitation care. Located at Chula Vista Center Mall on H Street, the building features imaging services, laboratory facilities, a pharmacy, and multiple specialty departments including cardiology, neurology and sports medicine. A 127,000-square-foot parking structure with 375 spaces connects to the medical centre via a second-floor pedestrian bridge. The project continues PMB's partnership with Sharp HealthCare dating back to the 1970s. HGW served as architect, with Pacific Building Group as general contractor. PMB has developed over 130 healthcare facilities totalling approximately 6.5 million square feet.
