- Complete needs assessments and care plans in a timely manner.
- Complete referrals and coordinate mental-health, housing, and other services.
- Facilitate discharge processes and locate housing for individuals.
- Complete discharge notes and communicate with community partners.
- Conduct warm handoffs for guests when appropriate.
- Maintain the proprietary customer relationship management database and partner contacts daily.
- Complete Coordinated Entry System documents and other compliance documentation.
- Complete Homeless Management Information System records.
- Use the bed-board application to determine census capacity for operational planning.
- Provide emergency-service referrals for domestic violence, human trafficking, and sexual assault.
- Attend care-coordination meetings and collaborate with the nursing team on weekly guest case conferences.
- Apply harm-reduction practices and conduct guest searches for contraband or illegal substances when applicable.
- Support guest interventions, including de-escalation, Crisis Prevention, and motivational interviewing.
- Conduct daily guest check-ins and build rapport and trust.
- Manage a caseload of at least 25 guests, with flexibility to manage additional guests as facility needs require.
- Complete home visits when necessary to track post-transition guest progress and Rapid Rehousing housing progress.
- Complete all required documentation during administrative hours.
- Collaborate with external case managers to provide services and support to guests.
- Triage and intervene during guest behavioral concerns.
- Follow funder and company operating standards, regulations, compliance requirements, best practices, and organizational policies and procedures.
- Adapt to schedule changes and complete additional assigned duties to meet facility needs.
- A master's degree in social work is preferred.
- Bilingual Spanish language ability is preferred.
