  • Experience conducting home visits and outreach (preferred)
  • Experience working with managed care patients (preferred)
  • Experience in customer- or client-service roles (preferred)
  • Knowledge of Cocoa, Florida Medicaid populations (preferred)
Responsibilities
  • Attend a 2-3-week long paid training program
  • Meet patients in the community, in the home, and in healthcare facilities and conduct a needs assessment
  • Help patients with health-related social issues
  • Create a directory of community resources
  • Manage relationships with healthcare facility providers
  • Expand knowledge of community resources and build ongoing relationships with organizations
  • Accompany members to medical appointments
  • Navigate technology systems to document patient encounters
  • Meet patients virtually for conversations
  • Support outreach to individuals eligible for Waymark support
  • Participate in weekly care team huddles
