Requirements
  • A Bachelor's Degree with two years of experience is required.
  • Relevant experience serving children and families in child welfare, developmental disabilities, behavioral health, primary health care, or social services is required.
  • Experience coordinating and participating in team settings is required.
  • Solid writing and verbal communication skills are required.
  • Ability to work effectively and knowledgeably across a broad spectrum of cultural, ethnic, and racial communities is required.
  • Ability to work in an effective and focused manner when self-directed and beyond immediate oversight is required.
  • Ability to deliver family-driven, youth-guided services is required.
  • Computer proficiency, including word processing skills and the ability to provide culturally competent practice, is required.
  • A valid driver's license meeting The Salvation Army insurance requirements is required.
  • Access to a vehicle to provide home visits is required.
Responsibilities
  • Assess, engage, and enroll clients into the health home program according to New York State Department of Health and Lead Health Home regulations.
  • Complete home visits with each enrolled member at least once per month, at a frequency determined by client need.
  • Collaboratively complete a yearly assessment of needs, strengths, and goals with the member, family, and care teams.
  • Facilitate at least a yearly meeting with members' care teams and work within the team to develop a comprehensive Plan of Care.
  • Provide care coordination, comprehensive care management, transitional care, crisis intervention, and caregiver and family support to enrolled members.
  • Coordinate with care teams, make referrals to other providers, ensure follow-up after hospital discharges and emergency department visits, and ensure preventive medical and dental care.
  • Document all case activities in an Electronic Health Record according to New York State documentation quality and time-frame guidelines.
  • Ensure monthly billing standards are met for each enrolled member and submit a billing questionnaire within the month in which the activity occurred.
  • Participate in mandatory and optional training.
  • Participate in weekly supervision.
