Requirements
  • Current New York State pharmacist license.
  • A Doctor of Pharmacy or Bachelor of Pharmacy degree.
  • Knowledge of pharmacotherapy and pharmacokinetics.
  • One year of experience in a hospital or infusion pharmacy is preferred.
  • Ability to communicate and collaborate effectively with patients, families, medical staff, and nursing staff.
Responsibilities
  • Provide and compound prescribed medications, intravenous mixtures, and other pharmaceuticals according to professional standards and practices.
  • Provide information and services necessary for the safe and appropriate use of drugs to patients, families, medical staff, and nursing staff to achieve optimal patient outcomes.
  • Review and interpret medication orders written by physicians or other authorized healthcare providers.
  • Verify medication orders for completeness, accuracy, and appropriateness in accordance with pharmaceutical care standards and prescribed policies and procedures.
  • Review patient profiles for drug interactions, drug allergies, therapeutic duplication, contraindications, drug-drug incompatibilities, dosing issues, and adverse drug reactions related to new orders or order changes.
