CNA Key Responsibilities:

· Transport residents safely to and from appointments including, but not limited to, physician appointments.

· Assist residents in and out of vehicles, community, and physician offices.

· Prepare list of scheduled appointments.

· When not transporting, perform CNA duties including, but not limited to, feeding of residents and assisting with activities as directed.

· Perform all duties as per SC Department of Public Health (DPH)

· Promote a cordial environment for residents, families, visitors and other co-workers; assist in maintaining a “home-like” environment

· Assist residents with ADLs and answer call lights

· Follow the residents care plan or service plan as an instruction guide for how to take care of that resident

· Report changes in residents' condition to the nurse in change and maintain communication with the families of residents

· Complete required documentation in accordance with community policies and state and/or federal regulations

· Maintain residents’ living area by ensuring that it is clean, neat and orderly, and in DPH compliance

· Comply with all safety and sanitary standards, rules and regulations

· Required to work weekends, holidays and per resident needs

· Perform other duties as assigned

CNA Requirements:

• High School Diploma or GED
• Certified Nursing Assistant 
• CPR Certification 
• 1-3 years of experience as a Certified Nursing Assistant
• Valid SC Drivers License with no infractions
