- An active Texas physical therapist license in good standing is required.
- The candidate must demonstrate strong clinical judgment and be comfortable working independently in the field.
- The candidate must be comfortable escalating changes in condition to registered nurses or physicians and documenting the rationale clearly.

- The candidate must be able to carry and transport basic therapy equipment as needed.
- The candidate must be competent in core home health physical therapy, including transfers, gait training, balance and fall prevention, assistive device recommendations, strengthening, functional mobility, home safety assessment and modification education, and patient and caregiver coaching with practical home programs.
- Own the caseload as a case manager by being accountable for each patient’s therapy plan, progression, and outcomes.
- Complete comprehensive in-home assessments and deliver skilled physical therapy visits.
- Build and execute plans of care covering gait and balance, transfers, strengthening, functional mobility, home safety, and fall prevention.
- Set measurable goals, track objective progress, and adjust treatment when patients do not progress.
- Coordinate with the multidisciplinary care team by communicating changes in condition, aligning on orders, and keeping care moving.
- Manage treatment and outcomes directly, including supervising physical therapist assistants where appropriate.
- Coach patients and caregivers on practical home programs.
- Document promptly and thoroughly in Homecare Homebase.
- Prior home health experience is preferred.
- Deeper clinical experience with high-signal physical therapy populations is preferred.
- Experience supervising or mentoring physical therapist assistants and collaborating on care plans is preferred.
- Strong organization and time management are preferred.
