Requirements

Responsibilities
  • Assist patients with making diet and nutrition choices that improve their quality of life.
  • Complete comprehensive nutrition assessments for all new and at-risk patients.
  • Collaborate with the interdisciplinary team and patients to develop and implement individualized care plans based on nutritional assessments, review them monthly, and document progress toward established goals.
  • Identify interventions necessary to meet nutritional needs based on evidence-based guidelines, best practices, and current research.
  • Educate patients and their families or caregivers about renal diets and fluid management.
  • Monitor monthly laboratory results and weight changes and make recommendations to meet established goals.
  • Use a patient-centered care approach that respects and responds to individual and cultural preferences while encouraging engagement.
  • Participate in the Quality Assessment Performance Improvement process, including documenting and attending meetings according to clinic policy.
  • Compile and create nutrition materials to promote adherence to renal diet guidelines.
  • Communicate nutrition concerns with the care team, including attending physicians.
  • Share advances in kidney disease and nutrition with the care team as needed.
