Requirements
  • An MS in Nursing is required.
  • An active national Nurse Practitioner certification is required.
  • A valid, current, active, and unrestricted Family or Adult Nurse Practitioner license in Florida is required.
  • A valid, current, active Registered Nurse license is required.
  • At least 2 years of experience managing complex care cases is required.
  • Experience working with Electronic Medical Records is required.
Responsibilities
  • Provide primary and urgent health care by telephone and tele-video to patients receiving home and community-based services through state Medicaid programs, dual eligible members, and other assigned memberships.
  • Develop and implement clinical plans of care for adult patients with chronic and complex conditions, including co-morbid medical and mental health diagnoses and limited personal resources.
  • Gather patient histories, perform physical examinations, and obtain diagnostics as needed before developing and implementing treatment plans consistent with patient goals and current conditions.
  • Identify and close gaps in care.
  • Address patients' and families' physical and psychosocial needs with support and input from the interdisciplinary team.
  • Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence, and crisis anticipation and prevention.
  • Coordinate care and resources with clinical team members, patients' other physicians, and other medical providers in a timely manner consistent with state regulations and company standards and policies.
  • Maintain patient medical records and medical documentation consistent with state regulations and company standards and policies.
  • Participate in continuing education as required by the state and certifying body.
  • Prescribe medication as permitted by state prescribing authority.
