Requirements
  • Float Position is a dialysis role requiring CCHT / Dialysis Experience.
  • Education: High School diploma or G.E.D. required.
  • Must meet CMS-approved state and/or national certification requirements within the required state or CMS timeline.
  • All appropriate state licensure, education, and training required.
  • Demonstrated commitment to organization culture, values, and customer service standards.
  • Continued employment is dependent on successful completion of the organization dialysis training program and successful CPR certification.
  • Must take the Ishihara's Color Blindness test as a condition of employment (failure does not preclude employment).
Responsibilities
  • Assist in education regarding adequacy measures where appropriate including Online Clearance Monitoring, Adequacy Monitoring Program, Urea Kinetic Modeling, and disease process/access.
  • Welcome assigned patients and inquire as to their wellbeing since their last treatment.
  • Evaluate vascular access for patency, perform vascular access cannulation, and administer heparin as delegated or as allowed by state law.
  • Obtain necessary pre and post treatment vital signs and weight and perform vascular access evaluation pre-treatment.
  • Initiate dialysis treatment according to prescribed orders including blood flow and dialysate flow.
  • Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
  • Monitor patients’ response to dialysis therapy.
  • Obtain vital signs prior to reinfusion; perform all relevant functions necessary for discontinuation of treatment and document.
  • Discontinue dialysis treatment according to established procedures and evaluate patient prior to termination of venous access.
  • Obtain hemostasis and apply appropriate dressings.
  • Evaluate the patient prior to discharge.
  • Record Pre and Post dialysis evaluation, weight, and vital signs with initial identification.
  • Calculate and enter dialysis machine programming for Ultrafiltration goal, treatment time, and UF modeling as prescribed.
  • Ensure safety alarms are enabled including Narrow Venous Limits 160.
  • Document treatment data accurately on Hemodialysis Treatment Sheet with initial identification.
  • Document and report significant information or changes in patient condition to RN or supervisor.
  • Observe patient and conduct machine safety checks; report changes or unusual findings.
  • Perform intervention for unusual patient status and document response.
  • Evaluate vascular access during treatment including arterial and venous monitoring pressures and document/report findings.
  • Obtain prescribed laboratory testing and prepare specimens for collection.
  • Ensure blood spills are cleaned with appropriate disinfectant.
  • Prepare and organize supplies and equipment to prevent wastage.
  • Monitor patients performing self-care under RN supervision.
  • Assist in training direct patient care staff under supervision of a nurse.
  • Follow organization policies, procedures and systems; comply with ethical business practices.
  • Maintain environmental integrity and aesthetics; ensure areas are safe and clean.
  • Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments; perform pretreatment alarm testing including Pressure Holding.
  • Initiate Solution Delivery System system.
  • Complete and document ongoing participation in Continuous Quality Improvement activities.
  • Enter all treatment data into designated clinical application accurately and timely.
  • Review treatment sheets for completeness and nursing signatures; correct omissions.
  • Prepare initial patient chart for admission and compile paperwork for chart placement.
  • Prepare/print lab requisitions for prescribed or stat Lab specimens; collect, label, prepare and store lab samples as required.
  • Ensure collection of lab specimens by appropriate lab courier.
  • Assist in infection control audits; support adherence to infection control practices; reinforce education of patients and families.
  • Follow infection control Policies and Procedures.
  • Participate in coordination of transitional dialysis care; liaison with staff to schedule disciplines; assist with home dialysis education; set and perform treatments on all machines used in transitional care unit; coordinate transition to patient’s modality choice; assist with supply ordering and inventory for Transitional Care Unit.
