Responsibilities
  • Conduct comprehensive assessments in patients' homes, including skilled documentation through Homecare Homebase.
  • Evaluate patients and home-health environments, perform skilled home-health assessments, complete clinical documentation and visit notes, develop care plans, and communicate with physicians, care-team members, patients, and families.
  • Treat wounds and perform infusions.
  • Serve as the patient's advocate and maintain ongoing communication with physicians, referral sources, caregivers, and facility management to coordinate care.
  • Administer medications and physician-prescribed treatments.
  • Provide direct patient care, including infusion therapy, wound care, teaching, and disease management.
  • Provide direct supervision of nursing clinical-care team members, including licensed vocational nurses and home health aides.
  • Participate in regular case conferences and in-service sessions.
Desired Qualifications
  • Previous home-health experience is highly preferred.
  • Experience with Homecare Homebase healthcare software is preferred.
