Requirements
  • Registered Nurse (RN) with a current, active license in California.
  • Minimum of 2 years in a clinical management role, with at least 1 year in hospice or palliative care.
  • Hospice and Palliative Care Certification (CHPN) preferred.
  • Proven ability to lead, motivate, and manage a team of clinical professionals in a fast-paced environment.
  • In-depth understanding of hospice care principles, symptom management, and end-of-life care.
  • Strong interpersonal, verbal, and written communication skills, with the ability to interact effectively with patients, families, and healthcare teams.
  • Excellent critical thinking skills, with the ability to make sound decisions in complex situations.
  • Proficient in electronic medical records (EMR) systems and other clinical software.
Responsibilities
  • Supervise, mentor, and provide guidance to a multidisciplinary team, including nurses, social workers, chaplains, and home health aides.
  • Ensure that all patients receive comprehensive, compassionate care that meets their individual needs and promotes comfort and dignity.
  • Monitor and assess the effectiveness of clinical interventions, ensuring adherence to hospice care protocols, regulatory guidelines, and patient safety standards.
  • Work closely with physicians, caregivers, and interdisciplinary team members to create and adjust individualized care plans for patients.
  • Ensure accurate and timely clinical documentation, including assessments, care plans, and progress notes, in compliance with regulatory and hospice standards.
  • Provide ongoing education and training for clinical staff on best practices in hospice care, including palliative symptom management, end-of-life issues, and family support.
  • Evaluate staff performance, provide constructive feedback, and support continuous professional development.
  • Serve as a liaison between families and the hospice team, advocating for the needs and concerns of both patients and their loved ones.
  • Be available for on-call duties as needed to address clinical issues or emergencies.
