Requirements
  • Graduation from an accredited school of nursing and an accredited nurse practitioner program, including preceptorship, with a Master of Science in Nursing required.
  • Certification by the applicable professional organization.
  • At least one year of clinical experience as a certified registered nurse practitioner within the last four years in a related primary care or ambulatory care setting.
  • Credentialing and continued good standing through the Veterans Health Administration.
  • A valid, unrestricted license in the state of employment or in any United States state or territory, depending on Veterans Affairs contractual requirements.
  • Current Basic Life Support certification and Advanced Cardiovascular Life Support certification as specified by the applicable Veterans Affairs medical center contract.
  • Current, unrestricted Drug Enforcement Administration registration.
  • Proficiency in written and spoken English.
  • Strong computer skills.
  • This is considered a safety-sensitive position.
Responsibilities
  • Provide prescribed medical treatment and personal care services to clinic patients with diseases and injuries under the direction of a physician or mid-level provider.
  • Collaborate with the core Primary Care PACT Team and expanded PACT Team to develop patient care plans.
  • Diagnose and treat veterans under the direction and responsibility of a supervising physician.
  • Examine patients for symptoms of organic or congenital disorders.
  • Develop and implement patient management plans and assist with continuity of care.
  • Order and perform diagnostic tests, including X-rays, electrocardiograms, and laboratory tests, and interpret results for deviations from normal.
  • Notify patients of test results and provide follow-up care.
  • Counsel patients on prescription medication use, educate patients, assess mental health issues, and provide routine health maintenance.
  • Evaluate records from medical providers outside the Veterans Affairs system and manage patients according to Veterans Affairs co-managed care rules.
  • Complete clinical reminders due at each patient visit.
  • Complete medical record documentation within 24 hours of each patient encounter.
  • Respond appropriately and timely to patient requests, alerts, notifications, consults, orders, laboratory results, and diagnostic studies in accordance with Veterans Affairs guidelines.
  • Follow the Veterans Affairs formulary process and consult protocols.
  • Complete all clinical encounters by close of business in accordance with Veterans Affairs documentation guidelines.
  • Complete Veterans Affairs and company training requirements.
  • Fulfill compliance requirements for the Office of Inspector General, Joint Commission, Environment of Care oversight, laboratory compliance, and related areas.
  • Remain focused on achieving clinical outcomes through specified Veterans Affairs guidelines.
  • Provide customer service to veterans, colleagues, and clinic visitors in person and by telephone.
  • Participate in clinic outreach events supporting enrollment initiatives.
  • Support patient enrollment and retention by guiding patients regarding scheduling, follow-up visits, and nurse visits.
  • Support new clinical and operational initiatives.
