- Maintain an active Texas occupational therapy license in good standing.
- Use strong clinical judgment and work independently in the field.
- Escalate changes in patient condition to a registered nurse or medical doctor and clearly document the rationale.
- Communicate effectively and maintain strong documentation practices.
- Reliably follow through on orders, visit frequencies, and patient goals.
- Hold a current driver’s license and travel daily across the assigned territory.
- Carry and transport basic therapy equipment as needed.
- Demonstrate competency in core home health occupational therapy scope, including activities of daily living and instrumental activities of daily living training, home safety, adaptive equipment, and caregiver education.
- Complete in-home occupational therapy evaluations and deliver skilled occupational therapy visits.
- Build and execute clear plans of care focused on activities of daily living and instrumental activities of daily living, safety and fall prevention, and functional independence.
- Recommend and train patients and caregivers on adaptive equipment and home modifications as needed.
- Coordinate with nursing and the broader care team when a patient’s condition or needs change.
- Document promptly and thoroughly in Homecare Homebase, using defensible, efficient, and consistent documentation.
- Prior home health experience.
- Experience with Homecare Homebase or a similar home health electronic medical record.
- Deeper experience with high-signal occupational therapy populations, including neurological conditions, cerebrovascular accident, fall risk, and dementia or caregiver training.
- Strong organization and time-management skills.
