  • Hold a current Texas Physical Therapist license.
  • Have at least one year of physical therapy experience.
  • Hold a current Texas driver's license.
  • Hold CPR certification.

Responsibilities
  • Perform comprehensive assessments in patients' homes, including skilled documentation through Homecare Homebase.
  • Evaluate patients and perform appropriate therapy interventions.
  • Develop comprehensive plans of care and coordinate multidisciplinary care.
  • Complete clinical documentation and visit notes promptly.
  • Communicate with physicians, care team members, patients, and families.
  • Work across all aspects of the home health setting.
  • Implement and maintain physical therapy protocols.
  • Manage patient treatment and outcomes directly or through Physical Therapist Assistant supervision.

  • Previous home health experience.
