- Hold a current Texas Physical Therapist license.
- Have at least one year of physical therapy experience.
- Hold a current Texas driver's license.

- Perform comprehensive assessments in patients' homes, including skilled documentation through Homecare Homebase.
- Evaluate patients and perform appropriate therapy interventions.
- Develop comprehensive plans of care and coordinate multidisciplinary care.
- Complete clinical documentation and visit notes promptly.
- Communicate with physicians, care team members, patients, and families.
- Work across all aspects of the home health setting.
- Implement and maintain physical therapy protocols.
- Manage patient treatment and outcomes directly or through Physical Therapist Assistant supervision.
- Experience with Homecare Homebase.
- Previous home health experience.
