  • RN or Licensed Vocational Nurse (LVN) required
  • 3 years Long Term Care experience
  • Specialized training in QAPI or QAPI Certification a plus
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
  • Ability to write reports, business correspondence, and procedure manuals
  • Ability to effectively present information and respond to questions from managers and employees
  • Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations
  • Proficient in Microsoft Suite products
  • Must maintain all CE for certifications and remain in good standing at all times with the State Board of Licensing
Responsibilities
  • Plan, organize, and direct quality assurance programs and activities
  • Develop, implement and coordinate QA and assessment programs to enhance resident care
  • Attend any required facility specific meetings/committees which support the facility QAPI program
  • Monitor trends and prepare quality reports for the QAPI committee
  • Develop processes in conjunction with the QAPI Committee to assess systems of care including clinical care, quality of life, and resident choice
  • Identify need for education based on findings and trends
  • Elicit data from facility identified systems to monitor care and services with findings compared to benchmarks and targets the faculty has established for performance
  • Work collaboratively with the Interdisciplinary team (IDT) to provide education/training based on findings
  • Track, monitor and investigate adverse events with identified action plan
  • Oversee Facility Infection control program which includes process of identification, reporting, surveillance, and investigation of communicable diseases and infections
  • Participate in correction of State and Federal survey deficiency corrections as appropriate
  • Attend facility in service training programs (i.e. HIPAA, Abuse Prevention, Infection Control, etc.)
  • Maintain the utmost level of confidentiality
  • Report suspected or known breaches
