Requirements
  • Knowledge of and compliance with state and federal pharmacy laws and regulations.
  • Knowledge of FDA health regulations, pharmacology, and legal compliance requirements for medication education and counseling.
  • Thorough understanding of dosage administration and measurement, chemical compounds, and medical brands.
  • Ability to organize and solve problems in situations involving uncertainty and rapid change.
  • Ability to align patient care activities and project work with departmental and institutional policies, business needs, goals, and vision.
  • Ability to collaborate with physicians, nurses, and other members of the healthcare team.
  • Ability to analyze, compare, and evaluate courses of action and make independent decisions.
Responsibilities
  • Demonstrate proficiency in pharmacist functions, including interpreting, transcribing, and verifying medication orders.
  • Attend and actively participate in daily multidisciplinary and bedside rounds.
  • Complete drug utilization reviews to promote rational drug therapy.
  • Lead and develop clinical services.
  • Mentor and educate staff to ensure accurate and appropriate documentation of clinical activities.
  • Review, interpret, and input treatment orders to ensure accurate dosage, treatment schedule, route of administration, and safe administration based on laboratory information and evidence-based practices.
  • Provide and support clinical pharmacy responsibilities by collaborating with hospital disciplines to support service lines.
  • Provide medication- and practice-related education and counseling to patients, caregivers, and medical staff.
  • Collaborate with medical and nursing staff to optimize pharmacotherapeutic aspects of patient care, including infusion and outpatient medication management.
  • Provide information to staff members and patients about medication uses, drug interactions, and potential side effects.
  • Oversee preparation of treatments, investigational treatments, prepackaging of medications, and drug returns.
  • Maintain records on patient prescriptions, charges, and inventory.
  • Ensure pharmacy technician duties are performed completely, safely, and without risk of harm to patients.
  • Precept pharmacy students and residents.
  • Perform other assigned duties.
