  • Graduate of an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
  • Bachelor's in Occupational Therapy, Master's in Occupational Therapy, or Doctorate in Occupational Therapy.
  • National certification by the American Occupational Therapy Association or eligibility for the American Occupational Therapy Association National Registration Examination.

Responsibilities
  • Provide occupational therapy evaluations and treatment plans to participants in accordance with applicable InnovAge policies and state regulations.
  • Provide clinical occupational therapy evaluation and treatment at acute, restorative, and maintenance levels to program participants.
  • Assess participants' conditions, create treatment plans, and discuss therapy implementation with participants or caretakers.
  • Integrate occupational therapy treatment plans into the team Plan of Care.
  • Assist with ordering, inventory, distribution, and maintenance of durable medical equipment for participants.
  • Make home visits and provide educational sessions as needed and indicated in the Plan of Care, documenting activities and participant or caregiver responses in participant charts.
  • Educate participants and family members on the proper care and use of supports, exercise programs, and durable medical equipment.
  • Administer treatment programs based on referrals and assessments from the interdisciplinary team.
  • Instruct family members, caregivers, and staff in methods to maximize safety and functional abilities when performing Activities of Daily Living, with documentation in medical record notes.
  • Evaluate charts and report participant progress to appropriate staff.
  • Document occupational therapy services and perform department record-keeping in accordance with documentation requirements.
  • Collect data, prepare reports, and maintain necessary participant statistics.
  • Assist in recommending occupational therapy policies and procedures.
  • Assist the physician or intermediate care provider in evaluating functional level, developing the plan of care, and revising it as necessary.
  • Participate in and present assigned in-service programs.
  • Participate with other healthcare personnel in patient care planning.
  • Perform skilled occupational therapy procedures ordered by a physician.
  • Write reports to physicians regarding patient progress.
  • Attend scheduled staff meetings and patient care conferences.
  • Complete discharge summaries upon completion of therapy services.
  • Prepare and submit patient evaluations, clinical notes, and progress notes in a timely manner according to agency policy.
  • Immediately report patient incidents, variances, or complaints.
  • Participate in peer review and quality management activities as requested.
  • Participate in utilization review of medical records as assigned.
  • Report incomplete work assignments to the appropriate clinical management staff member.
  • Collaborate with InnovAge nursing home and assisted living staff regarding the occupational therapy needs of resident participants.
  • Conduct and participate in clinic educational programs.
  • Maintain the therapy department in a clean and orderly manner and oversee therapy equipment maintenance.
  • Maintain quality assurance, safety, environmental, and infection control programs.
  • Fulfill documentation and department record-keeping requirements for occupational therapy services.
  • Attend morning meetings, mini teams, and family meetings, and forward information to the Primary Occupational Therapist when no other occupational therapist is available or when assigned.
  • Participate within the interdisciplinary team in formulating Plans of Care for InnovAge PACE participants and in other interdisciplinary team settings that plan, coordinate, and monitor participant care.
