Requirements
  • A high school diploma or equivalent is required.
  • Access to reliable transportation is required to facilitate travel throughout the service area and perform essential functions.
  • One or more years of experience within the last five years in an activities program in a healthcare setting serving a frail or elderly population is required, unless the candidate completes the Fallon Health training program.
  • CPR and Alzheimer's certifications, or willingness to obtain these certifications, are essential.
Responsibilities
  • Serve as a member of the PACE interdisciplinary team by assessing participants’ needs, interests, and capabilities and developing individualized therapeutic recreational care plans based on diagnoses and assessments.
  • Plan, organize, direct, and participate in a comprehensive therapeutic recreation program tailored to participants’ general and individualized needs and limitations.
  • Conduct large and small therapeutic recreational groups and individual recreational activities targeted to participants’ cognitive, physical, social, psychosocial, spiritual, and artistic needs.
  • Contribute activity ideas for monthly activity planning calendars.
  • Provide regular educational and culturally competent activities, including exercise, arts and crafts, music appreciation, and spirituality.
  • Ensure participant choice and engagement when planning and conducting activities.
  • Build and foster a sense of community and a warm social atmosphere at the Adult Day Health Center while treating participants with respect and dignity.
  • Maintain the cleanliness of recreational supplies and equipment according to procedure and notify the supervisor of damaged or unsafe equipment.
  • Suggest supplies and equipment that may benefit the activity program.
  • Participate as a member of the interdisciplinary team and attend assigned team meetings.
  • Participate in individual or family meetings and case conferences as assigned.
  • Inform the supervisor of changes in participants’ condition or complaints registered by participants.
  • Complete assigned work reliably and within identified time frames.
  • Document PACE center attendance daily as assigned.
  • Document participation in activities and other required records daily as assigned.
  • Demonstrate flexibility in meeting department needs.
  • Communicate and interact effectively and professionally with participants, coworkers, and external customers.
  • Understand, anticipate, and respond to participants’ needs within the scope of the role.
  • Attend staff and other assigned meetings.
  • Perform duties in accordance with Fallon Health and Summit ElderCare policies and procedures.
  • Complete activity assessments as needed and within timelines required by PACE regulations.
