Requirements
  • A minimum of two years of experience in a Residential Counselor role is required.
  • A demonstrated track record as an exceptional performer and team player while in a Residential Counselor role is required.
  • A high school diploma or equivalent is required; experience may substitute for academic training in some cases.
  • A valid driver's license is required, along with adequate insurance when using a personal vehicle, a safe driving record, at least six months of maintaining a valid United States driver's license, and the ability to pass a driver's screening background check.
  • Applicants must be at least 21 years of age.
  • Applicants must be able to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds using proper lifting techniques or a two-person lift, operate a computer and office equipment, sit, reach, climb stairs, and maneuver through narrow spaces or hallways.
  • Applicants must be able to assist clients with activities of daily living and communicate with clients, staff, and external representatives.
  • Applicants must have the specified close, distance, color, peripheral, and depth vision abilities and be able to adjust focus.
  • CPR certification is required within two weeks of hire.
  • First Aid certification is required within two weeks of hire.
  • Medication Administration Certification must follow Connecticut guidelines.
  • Safety Care certification is required within two weeks of hire.
  • Applicants must have knowledge of advocacy, empowerment, self-advocacy, participatory planning, physical personal management, household management, and crisis intervention techniques.
  • Applicants must be able to use Microsoft Outlook and an electronic health record.
  • Applicants must be able to work independently and as part of an interdisciplinary team, adapt communication styles, maintain confidentiality, organize work, multitask in a time-sensitive environment, and learn new skills.
Responsibilities
  • Assist in developing, implementing, and documenting treatment, including individual service plans, individual program plans, and clinical programs.
  • Provide training and support to persons served through rehabilitative and behavioral interventions, safety monitoring, appointment scheduling, money management, activities of daily living, grocery shopping, meal planning and cooking, household chores, and related activities.
  • Perform hands-on program tasks as required to implement support plans.
  • Assist individuals with community-based support and resource services and help them become integrated into their communities.
  • Plan recreational and social activities, outings, community travel, church activities, shopping, celebrations, and other activities that promote emotional well-being and quality of life.
  • Provide transportation as necessary.
  • Assess residents' physical and emotional status and report changes to a supervisor.
  • Monitor residents' funds as needed.
  • Maintain records, collect behavioral and academic data, maintain progress notes, enter information into the electronic health record, and document incidents, programs, and routine activities in communication logs.
  • Store, administer, document, and dispose of medication consistent with Medication Administration Certification requirements, funding-source regulations, and company policy.
  • Communicate with program nurses, funding agencies, case managers, other service providers, Visiting Nurses Association nurses, physicians, and other interested parties.
  • Participate in mandated training and staff meetings and assist with orientation of new staff.
  • Ensure compliance with internal and external regulatory requirements.
  • Ensure program-site safety and maintenance, including housekeeping, lawn mowing, snow shoveling, and garbage removal.
  • Follow corporate and divisional policies and procedures, safe work practices, workers' compensation requirements, universal precautions, and exposure-control procedures.
  • Complete paperwork and electronic records, including communication logs, community membership logs, chore schedules, timesheets, and application forms.
  • Ensure required staff-to-consumer ratios are maintained and provide last-minute coverage when required.
  • Serve as a human rights advocate, provide physical intervention or first aid during behavioral or medical emergencies, and report to the Disabled Persons Protections Commission.
  • Maintain required certifications, respond to work emails, and follow administrative policies.
  • Develop the overall quality of the program, model skills for program staff, and assist with staff training and onboarding.
  • Perform other related duties as required.
