Responsibilities
  • Act as a resource and provide answers to drug information questions for patients, family, pharmacists, nurses, physicians, medical students, undergraduate and graduate pharmacy and students and other associated hospital personnel.
  • Screen for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food in cooperation with the Dietetics Department, drug-disease state and drug-laboratory interactions.
  • Participate in appropriate hospital and/or departmental committees relevant to practice area.
  • Coordinate all aspects of patients' pharmaceutical needs in the patient care areas.
  • Duties include medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; coordination of patients' pharmaceutical needs with physicians and nurses.
  • Work with adult, adolescent, geriatric, and neonatal patients depending on practice location; demonstrate knowledge and skills to provide care appropriate to the age of the patients served; demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age.
  • Supervise Pharm.D. students, technicians, undergraduate and graduate student teaching, and provide educational programs for hospital staff (physicians, nurses and pharmacy staff)
