Requirements
  • A Registered Nurse currently licensed in the state where care is provided is required.
  • Previous hospice experience is preferred.
  • The candidate must maintain confidentiality of patient information and patient records.
  • The candidate must follow directions and adhere to defined care plans.
  • The candidate must demonstrate commitment to the hospice philosophy of care.
  • The candidate must have strong organizational and self-management skills.
  • The candidate must demonstrate critical thinking and good judgment.
  • The candidate must successfully pass background checks.
  • The candidate must have a valid driver's license.
Responsibilities
  • Receive patient assignments from the Patient Care Manager or Case Manager and coordinate care and resource use with attending physicians, hospice staff members, and other agencies.
  • Maintain communication with attending physicians concerning patient and family care.
  • Contact families regarding new patient assignments and maintain communication as requested or desired by the family.
  • Collect physical, psychological, social, and spiritual data affecting patient and family health status and incorporate it into the Plan of Care.
  • Maintain accurate and current patient records, documenting problems, plans, actions, goals, and changes, and submit required paperwork.
  • Instruct patients and caregivers on medication administration, side effects, adverse reactions, personal care, oral hygiene, lifting, moving, ambulation, exercise, disaster and safety management, and preparation for events preceding death.
  • Inform the Patient Care Manager of unusual or potentially problematic patient or family issues.
  • Attend hospice team meetings, patient conferences, and care plan meetings as needed.
  • Provide appropriate support at the time of death.
  • Share in providing 24-hour, seven-days-a-week coverage to patients and families.
  • Evaluate hospice aides every two weeks.
  • Evaluate LPNs at least once per month.
  • Develop understanding of hospice philosophy through individualized studies.
  • Develop proficiency in pain management through individualized reading, in-service training, or seminars.
  • Perform other assigned tasks.

  • Previous experience in hospice, home health, or geriatrics.
  • Hospice experience.
