- Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Licensure: Current state license as a Registered Nurse.
- Proof of current CPR certification.
- Current Driver's License.
- Experience: Two years’ experience as a Registered Nurse in a clinical setting and one-year full-time home health experience is required
- Knowledge of OASIS is required
- Skills: Strong assessment and clinical skills, excellent communication skills, ability to work independently and as part of a team.
- Familiarity with infection control measures (OSHA).
- Knowledge of State and Federal regulations.
- Good interpersonal skills.
- Home Care Home Base (HCHB) knowledge.
- Works in patients' homes in various conditions and locations.
- Assess home health patients utilizing the Nursing Assessment and LCD criteria to identify physical, psychosocial, and environmental needs.
- Ability to work a flexible schedule; ability to travel locally
- PRN emergency call availability.
- Develop and document the plan of care in correlation with the assessment.
- Provide nursing care utilizing infection control measures (OSHA).
- Work with the Interdisciplinary Group to ensure patient/family needs are met.
- Communicate with attending physicians, home health physicians, and other physicians involved in the patient’s care.
- Timely submission of all required paperwork.
- Demonstrate commitment, professional growth, and competency.
- Meet productivity standards.
- Perform on-call responsibilities and provide on-call service to patients/families as assigned.
- Provide effective communication to patient/family, team members, and other healthcare professionals.
- Transportation: Reliable transportation and valid driver's license. Valid and current auto liability insurance.
- Complete admissions for new home health patients on weekends.
- Administer nursing care for patients of all ages in their place of residence.
- Coordinate care with the interdisciplinary group, patient/family, and referring agency.
- Assume responsibility for coordination of care.
- Communicate with attending physicians, home health physicians, and other physicians involved in the patient’s care.
