Requirements
  • At least 2–3 years of experience in a primary care or value-based care setting working with Medicare Advantage, Medicare, and Medicaid patient populations.
  • An active and unrestricted Florida Advanced Practice Registered Nurse license.
  • A Drug Enforcement Administration license, or eligibility to obtain one within two months of the hire date.
  • Strong understanding of chronic disease management, preventive care, and quality metrics in a value-based care model.
  • Strong documentation and patient relationship-building skills.
  • Proficiency with electronic medical record systems; experience with eClinicalWorks is a plus.
Responsibilities
  • Provide personalized, preventive senior primary care.
  • Make autonomous clinical decisions and contribute to staffing decisions.
  • Manage chronic diseases and preventive care within a value-based care model.
  • Document patient care and build effective patient relationships.
  • Work Monday through Friday from 8:00 a.m. to 5:00 p.m. with no scheduled on-call or weekend shifts.

FORT WORTH, Texas--(BUSINESS WIRE)--Palm Primary Care Texas (“Palm”), part of the MBF Healthcare Partners II, L.P. (“MBF”) portfolio and Florida-based Palm Medical Centers, has acquired the assets of two primary care practices located in Dallas and Duncanville, from Texas Health Physicians Group. While the transaction has closed, the full transition of staff and clinics to the Palm name will occur over the next several months. With this acquisition, Palm has now opened or acquired six primary care clinics in Tarrant and Dallas counties since June 2023. The strategic expansion is a key part of having a meaningful impact on the health of Medicare beneficiaries throughout North Texas by 2025. The first clinic, located at 3920 W Wheatland Rd Ste 152 in south Dallas, is led by Bane Smith, MD and Eliazar Garcia Alvarez, MD, both of whom have served at the clinic for over 20 years
