Requirements
  • The candidate must hold a current Registered Nurse license.
  • The candidate must be a graduate from an approved school of nursing.
  • The candidate must have a minimum of two years of experience in an acute setting requiring heart, lung, and physical assessments, or demonstrated ability or education in those areas.

Responsibilities
  • The Registered Nurse manages and coordinates the care of assigned clients in clients' homes by providing comprehensive skilled nursing services and referrals for therapeutic services as needed to achieve maximum rehabilitation and recovery.
  • The nurse completes initial and periodic comprehensive assessments of clients, caregivers, and home environments for admission and continued agency services, determining needs, care effectiveness, and anticipated outcomes; assessments may include OASIS items at appropriate time points.
  • The nurse accepts assigned client admissions and weekend on-call assignments.
  • The nurse provides comprehensive nursing care using home assessments, nursing diagnoses, and physician-approved care plans.
  • The nurse performs treatments requiring specialized skills as ordered by the physician.
  • The nurse observes signs and symptoms and reports them to the VP of Clinical Services, Director of Professional Services, and/or physician as appropriate.
  • The nurse notes changes in clients' physical or emotional status.
  • The nurse assesses clients' nutritional needs and diets as ordered by the physician.
  • The nurse educates patients and families on medications and procedures.
