Ongoing clinical excellence requires someone monitoring and analyzing data. The support from our Quality Data Collection Specialist helps us maintain and improve our outcomes. It’s the sort of job that requires up-to-date knowledge of guidelines and requirements. You might feel like you’re only focused on data; in fact, you are focused on improving patient care.

WHAT YOU WILL DO
Identify and validate care gaps in provider appointments.
Enter gap in care in upcoming appointment note to provider.
Follow-up review after appointment to confirm gaps in care closed during visit.
Assist with closing quality gaps in care.
Review gaps in care report and validate care gaps by using health plan quality reports, Epic reports and health plan systems (i.e. Availity, Health BI).
Monitor gap closure by utilizing an excel work list.
Assist with year-end and annual quality audit.
Assist with coordinating PCP visits.
Outreach to members/beneficiaries to close gaps in care.
Mail educational information to members/beneficiaries regarding preventive care quality metrics.
Educate members/beneficiaries regarding preventive care and document as an encounter in Epic.
Assist with other focused quality initiatives.
WHAT IT TAKES TO SUCCEED
Work autonomously and be accountable in a complex environment.
Adapt within an ever-changing healthcare environment.
Strong communication skills.
Medicare and commercial insurance knowledge.
Knowledge of electronic health records.
