Requirements

Responsibilities
  • Identify and validate care gaps in provider appointments.
  • Enter gap in care in upcoming appointment note to provider.
  • Follow-up review after appointment to confirm gaps in care closed during visit.
  • Assist with closing quality gaps in care.
  • Review gaps in care report and validate care gaps by using health plan quality reports, Epic reports and health plan systems (i.e. Availity, Health BI).
  • Monitor gap closure by utilizing an excel work list.
  • Assist with year-end and annual quality audit.
  • Assist with coordinating PCP visits.
  • Outreach to members/beneficiaries to close gaps in care.
  • Mail educational information to members/beneficiaries regarding preventive care quality metrics.
  • Educate members/beneficiaries regarding preventive care and document as an encounter in Epic.
  • Assist with other focused quality initiatives.
Desired Qualifications
  • 1 year Clinical Experience Preferred
  • 1 year Medical Assistant Preferred
