Requirements
  • A Certified Nursing Assistant credential or experience as a caregiver.
  • First Aid and cardiopulmonary resuscitation certification.
  • Completion of state regulatory certifications and ongoing training requirements.
  • At least six months of experience in a related position.
  • Compliance with licensing requirements.
  • Experience with Yardi, electronic medication administration records, Schedule Pop, ADP, or other relevant platforms.
Responsibilities
  • Prepare and assist with medications as prescribed by healthcare providers for residents in Assisted Living and Memory Care.
  • Document medications given and not given in the electronic medication administration record.
  • Communicate with residents, healthcare providers, families or powers of attorney, and the director or nurse about medication-order changes, refill challenges, and missed scheduled doses.
  • Observe, report, and document changes in resident conditions in Yardi electronic health record progress notes and the Nurse to Med Aide Communication Log.
  • Document and follow up on resident concerns in the End of Shift Report.
  • Reorder and receive medications, ensure residents do not run out, and confirm medications are logged, properly labeled, and stored.
  • Notify the director or nurse of medication discrepancies.
  • Provide first aid when needed and respond to medical emergencies.
  • Assist with internal incident reports and data entry into Yardi electronic health records.
  • Document alert charting once per shift in Yardi electronic health record progress notes.
  • Document monthly weights and vital signs as needed in Yardi electronic health records.
  • Maintain clean and organized medication rooms, medication carts, and medication overflow areas.
  • Complete a narcotic count with the oncoming shift.
  • Destroy medications according to policy.
  • Participate in weekly medication audits.
  • Respond to resident pendant calls as needed.
  • Assist residents with activities of daily living according to their personalized service plans.
  • Collaborate with caregivers to provide resident care and recommend ways to improve quality of care.
  • Follow infection-control procedures.
  • Maintain the confidentiality of resident and family information.
  • Follow company policies and procedures.
  • Complete monthly training assignments as scheduled or assigned by the director.
  • Perform other assigned duties.
