  • High school diploma or equivalency
  • Minimum of one (1) year of experience in the healthcare or hospitality industry in a patient-facing role with emphasis on patient satisfaction
  • Strong technical skills within all Microsoft Suite applications, electronic medical record systems, and productivity tools
  • Excellent leadership, organizational, and cross-collaborative communication skills in settings with seniors, their families, and healthcare providers
  • Reliable means of transportation
  • Bilingual in Spanish preferred
Responsibilities
  • Provide high-quality and excellent customer service to all enrolled PACE participants
  • Act as a liaison and maintain open communication with different disciplines, and internal and external providers, to ensure the highest level of care
  • Assist with the grievance process, providing high-touch customer service and partnership with the interdisciplinary team (IDT), Quality Improvement department, and participant until resolution
  • Maintain tracker to identify immediate needs and follow up
  • Attend & lead interdisciplinary care team meetings, Participant Council, and any other meetings as directed
