Requirements
  • A high school diploma or General Educational Development (GED) credential is required.
  • Meet Centers for Medicare and Medicaid Services-approved state and/or national certification requirements within the required state or CMS timeline.
  • Hold all appropriate state licensure, education, and training required for the role.
  • Complete the organization’s dialysis training program successfully for continued employment.
  • Complete cardiopulmonary resuscitation (CPR) certification successfully for continued employment.
  • Demonstrate commitment to organizational mission, culture, core values, and customer service standards.
  • Pass the Ishihara Color Blindness test as a condition of employment, with reasonable accommodations considered if needed.
  • Meet the physical requirements of direct patient care, including heavy lifting and moving patients, assisting with ambulation, prolonged standing, bending, and lifting solutions up to 40 pounds.
Responsibilities
  • Reinforce home hemodialysis and/or peritoneal dialysis training as permitted by law.
  • Welcome patients, obtain vital signs and weights, and gather information for the Clinic Visit Intake form.
  • Maintain communication and collaboration with patients, care partners, nurses, staff, providers, and other team members.
  • Obtain treatment records, review them for completion, and report missing or abnormal data to the home therapies registered nurse.
  • Assist with electronic home treatment, medication, and oral nutritional supplement reconciliation.
  • Assist home therapies registered nurses with home visits and provide permitted home support.
  • Prepare supplies and specimens for prescribed laboratory testing and shipping.
  • Track and ship transplant-related laboratory specimens and reinforce transplant education.
  • Complete opening and closing chores and document them appropriately.
  • Disinfect dialysis machines, chairs, tables, and other equipment between patient visits and routinely.
  • Prepare exam and training rooms for home dialysis training, treatment, and other visits.
  • Stock exam, training, treatment, and storage rooms with supplies.
  • Manage supplies to maximize use and minimize waste.
  • Schedule patient appointments and contact patients or caregivers about dates, times, and reminders.
  • Support patient registration and use of connected health.
  • Assist patients and care partners with supply and inventory ordering and management.
  • Maintain logs and tracking records.
  • Order and track clinic or office supplies as directed.
  • Participate in medical records, infection control, and other audit processes as directed.
  • Participate in staff huddles and meetings.
  • Collect required patient admission documentation and prepare and maintain electronic or hard-copy patient charts.
  • Perform clerical duties such as answering phones, faxing, email, and contacting provider offices.
  • Set up, tear down, program, test, and disinfect the home hemodialysis machine as directed.
  • Troubleshoot home hemodialysis machinery, tools, and connectivity issues.
  • Prepare dialysate and dialysis supplies for home hemodialysis treatment according to standing or provider orders.
  • Evaluate dialysis access before treatment, document findings, and report abnormalities to the home therapies registered nurse.
  • Initiate home hemodialysis treatment while monitoring and documenting patient status, vital signs, and machine data.
  • Perform and document treatment safety checks at intervals designated by the Centers for Medicare and Medicaid Services and facility policy.
  • Report significant changes in patient status during treatment and document them in the medical record.
  • Provide directed and legally permissible interventions during treatment in response to interdialytic events and document them.
  • Obtain and document pre-treatment termination vital signs.
  • Ensure hemostasis of the dialysis access before leaving the treatment area.
  • Collect and document post-treatment vital signs and report them to the home therapies registered nurse.
  • Assist with quarterly, semi-annual, and annual water and dialysate testing as directed.
  • Troubleshoot peritoneal dialysis cycler, modem, and connectivity issues.
  • Perform phlebotomy for prescribed laboratory testing as directed and when state and competency requirements are met.
  • Set up the peritoneal dialysis cycler and prepare and warm peritoneal dialysis solutions as directed.
  • Assist with exit-site care as directed.
  • Track peritoneal dialysis equipment as needed.
  • Travel between assigned facilities and locations within the community, with regional, business unit, and corporate meeting travel as required.
