Requirements
  • A nursing degree from an accredited college or university or graduation from an approved Licensed Practical Nurse or Licensed Vocational Nurse program is required.
  • A current, unencumbered, active license to practice as a Licensed Practical Nurse or Licensed Vocational Nurse in the state is required.
  • Current Cardiopulmonary Resuscitation certification is required.
  • All required continuing education and licensing must be maintained.
  • The employee must remain in good standing with the State Board of Nursing.

Responsibilities
  • Direct the day-to-day functions of nursing assistants in accordance with the rules, regulations, and guidelines governing the long-term care facility.
  • Ensure that assigned nursing personnel comply with the facility’s written policies, procedures, job descriptions, and Nursing Service Procedures Manual.
  • Review department policies, procedure manuals, and job descriptions and recommend revisions.
  • Plan shift services, programs, and activities with assigned nursing staff and support personnel.
  • Maintain the Nursing Service Procedures Manual so that it reflects daily nursing procedures.
  • Prepare written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident’s total care regimen.
  • Participate in the development, maintenance, and implementation of the nursing service quality assurance program.
  • Participate in authorized government facility surveys and inspections when requested.
  • Review and update residents’ written discharge plans and assist with planning nursing services for discharge.
  • Interpret department policies and procedures for personnel, residents, visitors, and government agencies.
  • Admit, transfer, and discharge residents as required.
  • Complete accident and incident reports and chart related incidents involving residents.
  • Complete medical forms, reports, evaluations, studies, charting, and other administrative records.
  • Maintain resident confidentiality, protect health information, and report unauthorized access attempts or disclosure violations.
  • Complete required records and charts upon resident admission, transfer, and discharge.
  • Receive telephone orders from physicians, record them, and transcribe physician orders to resident charts, medication cards, and treatment or care plans.
  • Chart nurses’ notes descriptively to reflect care provided and the resident’s response.
  • Record diet orders and forward diet changes to the Food Services Department.
  • Report discrepancies involving physician orders, diet changes, charting errors, and other documentation issues to the Nurse Supervisor.
  • Prepare and administer physician-ordered medications and treatments after verifying resident identity.
  • Maintain adequate supplies of floor-stock medications, medical supplies, and equipment and report needs to the Nurse Supervisor.
  • Order prescribed medications, supplies, and equipment in accordance with established policies.
  • Maintain accurate narcotic records and report discrepancies to the Nurse Supervisor.
  • Review medication cards for completeness, order-transcription accuracy, and compliance with stop-order policies.
  • Notify physicians about automatic stop orders before the final dosage is administered and dispose of drugs and narcotics according to procedure.
  • Participate in employee performance evaluations, determine shift staffing requirements, and recommend dismissals or transfers to the Nurse Supervisor.
  • Report staffing needs and absentee call-ins to the Nurse Supervisor.
  • Develop work assignments and provide leadership to nursing personnel assigned to the unit or shift.
  • Make daily rounds to ensure nursing personnel perform assignments according to acceptable nursing standards and report problem areas.
  • Ensure nursing assistants are enrolled in or have graduated from an approved training program and that nurse aide trainees work under the direct supervision of a licensed nurse.
  • Meet regularly with shift nursing personnel to identify and correct problem areas and improve services.
  • Review personnel complaints and grievances and make required reports to the Nurse Supervisor.
  • Administer departmental disciplinary action fairly and without discrimination.
  • Receive and give nursing reports at the beginning and end of shift duty hours.
  • Report occupational exposures, suspected fraud, and other incidents according to facility procedures.
  • Inform nursing personnel about new admissions and prepare rooms for new residents.
  • Greet and orient newly admitted residents and their family members to the facility.
  • Make physician rounds and consult with residents’ physicians regarding care, treatment, and rehabilitation.
  • Requisition and arrange physician-ordered diagnostic and therapeutic services.
  • Review resident charts for treatments, medications, diets, and other orders.
  • Implement and maintain nursing objectives and standards.
  • Check that prescribed treatments are properly administered and evaluate residents’ physical and emotional status.
  • Provide or ensure direct nursing care in accordance with staff qualifications and resident care plans.
  • Coordinate social and activity programs with nursing service schedules.
  • Notify physicians and next of kin about accidents, incidents, and changes in residents’ conditions.
  • Carry out restorative and rehabilitative programs, including self-help and care.
  • Inspect nursing treatment areas daily for cleanliness and safety.
  • Provide professional nursing services including catheterization, tube feeding, suctioning, dressing and bandage changes, ostomy and drainage-bag care, blood collection, massage, range-of-motion exercises, and care for dying residents.
  • Use restraints when necessary and according to policy.
  • Collect sputum, urine, and other specimens for ordered laboratory tests.
  • Take and record temperature, pulse, respirations, blood pressure, and other measurements.
  • Monitor seriously ill residents.
  • Check food brought by residents’ families or visitors against residents’ dietary allowances.
  • Ensure direct care follows residents’ care plans and wishes and that residents unable to call for help are checked frequently.
  • Meet with residents and family members and report care concerns to the Nurse Supervisor.
  • Arrange transportation and escort discharged residents to the pickup area.
  • Notify family members of residents’ deaths, contact funeral homes when requested, and ensure post-mortem procedures are followed.
  • Develop, plan, conduct, and schedule in-service training classes for nursing service personnel.
  • Implement and maintain an orientation program for new employees.
  • Assist in standardizing work methods and training personnel on tasks involving potential exposure to blood or body fluids.
  • Assist with clinical supervision planning for nurse aide trainees.
  • Attend outside training, annual facility in-service training, advance-directive training, and continuing education programs.
  • Monitor personnel compliance with safety regulations, departmental procedures, dress codes, and equipment requirements.
  • Identify and document tasks involving potential exposure to blood or body fluids and ensure required training before such tasks are performed.
  • Maintain adequate personal protective equipment and ensure its availability to personnel.
  • Maintain nurses’ stations, medication rooms, resident care rooms, and treatment areas in a clean, safe, and sanitary condition.
  • Ensure compliance with handwashing, hand hygiene, infection-control, isolation, and personal protective-equipment disposal procedures.
  • Ensure participation in fire-safety and disaster-preparedness drills.
  • Participate in developing, implementing, and maintaining the infection-control program.
  • Develop and maintain procedures for reporting hazardous conditions or equipment.
  • Ensure personnel use safety equipment, including back braces and mechanical lifts, when moving residents.
  • Report missing or illegible hazardous-chemical labels and Safety Data Sheets.
  • Recommend unit equipment and supply needs to the Nurse Supervisor.
  • Maintain adequate medication, medical supply, and equipment stock levels.
  • Participate in procedures for safe operation of nursing-service equipment and ensure that only trained, authorized personnel operate it.
  • Monitor efficient use of nursing supplies and ensure Safety Data Sheets are maintained for hazardous chemicals.
  • Review resident care plans daily and report needed changes to the Nurse Supervisor.
  • Ensure nurses’ notes document compliance with care plans.
  • Review care plans for appropriate goals, problems, approaches, and revisions.
  • Ensure certified nursing assistants know and follow resident care plans.
  • Assist with planning, scheduling, and revising the Minimum Data Set, including implementation of Resident Assessment Protocols and Triggers.
  • Report suspected or known fraud involving false billings, cost reports, or kickbacks.
  • Provide requested data to the Quality Assurance and Assessment Committee.
