Requirements
  • Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
  • Must possess a current, unencumbered, active license to practice as an LPN/LVN in this state.
  • Must be able to read, write, speak, and understand the English language.

Responsibilities
  • Provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants.
  • Direct the day-to-day functions of nursing assistants in accordance with applicable rules, regulations, and guidelines.
  • Ensure that nursing personnel comply with facility policies, procedures, job descriptions, and the Nursing Service Procedures Manual.
  • Plan shift services, programs, and activities with assigned nursing and support personnel.
  • Make written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident’s total care regimen.
  • Participate in the nursing service quality assurance program and facility surveys conducted by authorized government agencies.
  • Review and update resident discharge plans and assist with planning nursing services for discharge.
  • Admit, transfer, and discharge residents as required.
  • Complete medical forms, reports, evaluations, studies, charting, accident and incident reports, census reports, charge slips, and other required administrative documentation.
  • Protect resident and facility information by maintaining the confidentiality of user credentials and protected health information and reporting unauthorized access attempts.
  • Complete and file records and charts for resident admissions, transfers, and discharges.
  • Receive telephone orders from physicians and transcribe physician orders to resident charts, medication cards, treatment plans, and care plans.
  • Chart nurses’ notes and accident or incident reports accurately and document the care provided and resident response.
  • Record diet changes and report discrepancies in physician orders, diet changes, or charting to the Nurse Supervisor.
  • Prepare and administer medications as ordered by physicians.
  • Verify resident identity before administering medications or treatments and prevent medication errors.
  • Maintain adequate medication, supply, and equipment stock and report supply needs.
  • Maintain accurate narcotic records, report discrepancies, review medication cards, manage automatic stop orders, and dispose of drugs and narcotics according to procedure.
  • Participate in employee performance evaluations, determine shift staffing requirements, and recommend employee dismissals or transfers to the Nurse Supervisor.
  • Report staffing shortages and absentee call-ins and review the workforce to recommend improvements.
  • Develop work assignments and provide leadership to nursing personnel assigned to the unit or shift.
  • Make daily rounds to ensure nursing personnel perform assignments according to nursing standards.
  • Ensure nursing assistants are enrolled in or have graduated from approved training programs and that nurse aide trainees work under direct licensed-nurse supervision.
  • Meet regularly with shift nursing personnel to identify and correct problem areas and improve services.
  • Review personnel complaints and grievances and report or address them according to facility procedures.
  • Receive and give nursing reports at the beginning and end of shift duty.
  • Report occupational exposures, fraud, information-security issues, and other incidents according to facility procedures.
  • Inform nursing personnel about new admissions and prepare rooms for new residents.
  • Greet and orient newly admitted residents and family members to the facility.
  • Make rounds with physicians and consult with residents’ physicians regarding care, treatment, and rehabilitation.
  • Arrange diagnostic and therapeutic services ordered by physicians.
  • Review resident charts for treatments, medication orders, diets, and other care requirements.
  • Implement and maintain nursing objectives and standards and verify that prescribed treatments are administered properly.
  • Assess residents’ physical and emotional status and notify physicians and next of kin of accidents, incidents, or changes in condition.
  • Coordinate social and activity programs with nursing schedules.
  • Carry out restorative and rehabilitative programs, including self-help and care.
  • Inspect treatment areas and ensure resident care areas are clean and safe.
  • Perform nursing services including catheterization, tube feedings, suction, dressing changes, colostomy and drainage-bag care, blood collection, massages, range-of-motion exercises, and end-of-life care.
  • Use restraints when necessary and according to policy.
  • Obtain specimens for laboratory testing and record temperature, pulse, respirations, blood pressure, and other measurements.
  • Monitor seriously ill residents and verify that resident care follows care plans and resident wishes.
  • Check foods brought by families or visitors against residents’ dietary allowances.
  • Meet with residents and family members and report concerns to the Nurse Supervisor.
  • Arrange transportation for discharged residents and escort them to the pickup area.
  • Inform family members of resident deaths, contact funeral homes when requested, and follow post-mortem procedures.
  • Develop, plan, conduct, and schedule in-service training classes.
  • Orient new employees to the shift, its policies, procedures, duties, and job positions.
  • Assist with standardizing work methods, blood and body-fluid exposure training, and clinical supervision for nurse aide trainees.
  • Attend outside training, annual facility in-service training, advance-directive training, and continuing education programs.
  • Monitor personnel compliance with safety regulations, dress codes, infection-control procedures, hand hygiene, and use of safety equipment.
  • Maintain adequate personal protective equipment and ensure safe handling and disposal of equipment and hazardous materials.
  • Participate in infection-control, fire-safety, disaster-preparedness, and hazardous-condition reporting programs.
  • Maintain clean, safe, and sanitary nursing work areas and resident care areas.
  • Recommend unit equipment and supply needs and maintain adequate medication, medical supply, and equipment stock.
  • Ensure only trained and authorized personnel operate nursing equipment safely and efficiently.
  • Maintain material safety data sheets for hazardous chemicals used by the nursing department.
  • Review resident care plans daily and report needed changes.
  • Ensure nurses’ notes reflect implementation of care plans and ensure certified nursing assistants follow resident care plans.
  • Assist with planning, scheduling, and revising the Minimum Data Set, Resident Assessment Protocols, and Triggers.
  • Provide data to the Quality Assurance and Assessment Committee as requested.

PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman.
