Responsibilities
  • Facilitate all aspects of the Stroke and Stemi Program.
  • Discuss the program with physicians, nurses and leadership team.
  • Assess provision of care to determine if evidence-based guidelines are met.
  • Initiate performance improvement activities when indicated.
  • Oversee education provided to patients and family members.
  • Provide staff education.
  • Serve as a resource for patients and their families after discharge.
  • Coordinate with AHMC Seton Medical Center Quality Department and corporate to support the position.
  • Support the goals of the Medical Center in advancing quality STEMI and stroke care and obtain/maintain Chest Pain Accreditation through the American College of Cardiology and Stroke Accreditation through The Joint Commission.
  • Coordinate care for patients through multidisciplinary teams to deliver high-quality care and reduce adverse events/readmissions.
  • Educate and mentor peers; comfortable teaching.
