Requirements
  • Be a licensed Registered Nurse in good standing and currently licensed in the state of practice and the state of residence.
  • Have less than two years of clinical experience and at least one year in a related management or supervisory role in home health care.
  • Hold current CPR certification.
  • Meet all agency requirements for pre-employment as required by the company and/or state regulations.
  • Be able to use company documentation systems.
  • Be able to communicate verbally and in writing with all levels of personnel, both internal and external to the company.
  • Be able to work independently as well as part of a team.
  • Be capable of working responsibly with highly confidential information.
Responsibilities
  • Conduct or participate in quarterly quality improvement meetings, review pertinent available data, identify trends, and oversee the implementation of improvement plans.
  • Collaborate with the Branch Director to implement quality improvement activities and corrective action plans.
  • Oversee and respond to external and internal surveys and audits within the assigned area relating to quality of care and clinically related key performance indicators, in collaboration with operations leadership.
  • Complete documentation and chart reviews to identify variances in standardized care and care processes and areas for improved performance related to patient care and the organization.
  • Hold case conferences to ensure oversight of care, coordination of services, and compliance with standards.
  • Facilitate the development of performance improvement action plans based on available data, including survey results, quality assessment and performance improvement data, and clinical record reviews.
  • Communicate job expectations; plan, monitor, and appraise job results; coach, counsel, and discipline employees; and develop, coordinate, and enforce systems, policies, procedures, and productivity standards.
  • Ensure assigned staff are evaluated fairly and timely in accordance with company performance evaluation procedures.
