Requirements
  • Graduate of an accredited school of nursing.
  • Hold a valid license to perform practical nursing in the Commonwealth of Massachusetts.
  • Have access to reliable transportation to perform work throughout the PACE center’s 30-mile service area when needed.
  • Be willing to occasionally assist other Summit ElderCare PACE centers in person or remotely when there is an opening or gap in coverage.
  • Have at least three years of nursing experience in a primary care practice, long-term care facility, or similar clinical setting.
  • Have at least one year of experience working in a nursing capacity with frail elders.
Responsibilities
  • Implement plans of care formulated by the Provider and Primary Registered Nurse in collaboration with the Interdisciplinary Team.
  • Receive participant and caregiver calls, triage each call appropriately, conduct follow-up, and document the interaction promptly in the medical record.
  • Administer prescribed medications to participants attending the PACE center.
  • Record vital signs and carry out assigned treatments and wound care for participants attending the day center.
  • Make frequent home visits in the community to evaluate the home environment.
  • Fill weekly medication packs.
  • Record vital signs and administer prescribed treatments during home visits.
  • Provide relevant health teaching to participants and their caregivers.
  • Make recommendations to the Interdisciplinary Team regarding unmet participant needs.
  • Coordinate participant care with the nursing supervisor and other nursing staff.
  • Travel occasionally or frequently throughout the service area to participants’ homes and contracted facilities.
