Requirements
  • Current certification as a Nurse Practitioner (NP) or Physician Assistant (PA)
  • At least 2 or more years of experience in outpatient cardiology setting required
  • Strong clinical knowledge and the ability to manage complex cardiac conditions
  • Excellent communication skills and the ability to work well within a multidisciplinary team
  • Compassionate, patient-focused attitude with a commitment to providing high-quality care
Responsibilities
  • Perform comprehensive patient assessments and diagnostic evaluations related to heart failure
  • Develop and implement individualized care plans, including pharmacologic and non-pharmacologic interventions
  • Monitor patient progress, adjust treatment plans as needed, and provide ongoing education to patients and families
  • Collaborate with cardiologists, nurses, and other healthcare professionals to provide coordinated care
  • Order and interpret diagnostic tests, including echocardiograms, EKGs, and lab work
  • Provide patient education regarding heart failure management, lifestyle modifications, and medication adherence
  • Document all patient interactions and maintain accurate and up-to-date medical records
