Requirements
  • A high school diploma or GED is required.
  • A valid Michigan driver's license is required.
  • A current CPR card is required.
  • Experience must be in a long-term care, hospital, home care, or PACE setting.
  • Completion of a basic nursing assistant training program is required, with a certificate of completion from a formal training program such as Direct Care, Nurse Assistant, or Medical Assistant outside of PACE SEMI.
  • Participation in annual mandatory in-service training and screening, including infection control, tuberculosis testing, safety training, and basic life support training.
  • Ability to establish and maintain interpersonal and interdepartmental relationships.
  • Ability to problem-solve and interpret instructions.
  • Ability to organize and maintain flexible scheduling to coordinate job responsibilities.
  • Ability to work sensitively with individuals of diverse ethnic and cultural backgrounds.
  • Ability to relate to participants with dementia or developmental disabilities.
  • Ability to perform duties in an atmosphere of frequent interruption.
  • Must meet a standardized set of competencies approved by the Centers for Medicare & Medicaid Services before working independently.
  • Must be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
  • Reliable transportation available daily, a current valid driver's license, an acceptable driving record, and proof of current automobile insurance are required.
  • Ability to transfer and assist with moving patients; frequent walking, bending, and lifting of forty pounds or more may be required.
Responsibilities
  • Provide supportive services to participants in the community setting, during transport and appointments, and in the day health center under the supervision of the manager, registered nurse, or lead Community Living Associate.
  • Perform care and services outlined in each participant's care plan.
  • Provide personal care, including two-person assistance when needed.
  • Provide one-to-one interaction in the day health center and community living setting as outlined in the plan of care.
  • Assist with activities, meal preparation, cooking, feeding, and other nutritional needs.
  • Assist with toileting and managing incontinence.
  • Provide simple treatments.
  • Perform grocery shopping, laundry, homemaking, chores, housekeeping, and equipment sanitization.
  • Coordinate care with participants and the day health center based on the care plan.
  • Engage with residents and appropriately elevate health concerns and customer service issues or needs.
  • Conduct rounds every two hours from 7:00 p.m. to 7:00 a.m.
  • Document care and services provided during participant encounters accurately and timely, and keep the manager, lead Community Living Associate, and clinical team informed of participant progress.
  • Immediately report customer service complaints and deviations from set schedules and appointments to a supervisor, manager, or lead Community Living Associate.
  • Respond to emergency situations in accordance with established policies and procedures.
  • Work only within the established scope of practice.
  • Participate in skills check-off performance evaluations, competency in-services, practicum demonstrations, written assessments, mandatory in-service training, and department staff meetings.
  • Drive within the PACE SEMI catchment area.
  • Adapt participant assignments based on changing acuity levels, participant needs, and participant census.
Desired Qualifications
  • Certification is optional but preferred.
  • One year of experience with a frail or elderly population.
