  • Master’s preferred, Bachelor’s required
  • Social Work, Community Development, Strategic Partnerships
  • Minimum 5 years of experience
  • Advanced communication skills including ability to use varied tech platforms. Including but not limited to Slack, Google Suite (Gmail, Docs, Sheets, Slides, etc.) and Mac products
  • Knowledge of electronic health record systems and/or care facilitation platforms preferred
  • Strategic thinker, cross-functional collaborator and ability to problem solve efficiently
  • Growth and learning mentality, ability to think outside the box, go outside the bounds of “traditional” responsibilities
  • Adaptable to change and prepared for frequent, fast-paced changes and shifting priorities
Responsibilities
  • Support local partners, relationships in accordance with central CBH social care priorities
  • Support creation of local framework for partnerships; terms of engagement to enable accountability and build preferred social care networks in market
  • Unlock value of social care benefits and set priorities in collaboration with payers for community engagement that is high yield for CBH members including strategic, in-market payer events
  • Create market framework and operational plan for engaging providers/care delivery organizations
  • Set market targets and success metrics
  • Collaborate with marketing to create talking points, pitch material, and operational plan
  • Align with company-wide care model design work
  • Adapt to local context and market-specific rules; work with leadership, CTLs and CHPs
