- A Bachelor's or Master's degree in social work from a school of social work accredited by the Council on Social Work Education.
- A Licensed Social Worker credential in good standing.
- At least one year of social work experience in a health care setting.
- Provide social-work services in a home health and hospice setting.
- Work with clients and provide family counseling.
- Participate in multidisciplinary health care settings.
- Provide on-call coverage some evenings and possibly weekends.
- At least two years of experience in varied multidisciplinary health care settings.
- Experience working with clients and in family counseling.
