Responsibilities
  • Monitor and complete new custom form requests on our Plus platform
  • Validate form request completeness (content files, instructions, required metadata), and flag compliance exceptions (e.g., no credit-card fields/PCI, scope constraints) and request clarifications
  • Manage content and files, organizing client forms and tracking progress centrally
  • Build forms in the Custom Form Builder (sections, fields, field types, logic/branching, required/optional rules), implementing signatures, and standardizing as needed
  • Update backlog counts; report on progress and aging items
  • Communicate status in tickets and to the Forms Lead
  • Ensure HIPAA compliance; document known limitations and approved workarounds
