- Manage a caseload of members with mixed acuity.
- Review and obtain member data and enter it into HealthMap's Compass care management documentation system.
- Complete member health and social determinants of health screenings.
- Perform medication reconciliation.
- Create and maintain member-centric care plans, including updates to identified problems, barriers, interventions, and goals.
- Collaborate with physicians, nurses, and other healthcare personnel to support positive patient outcomes and care coordination.
- Handle inbound and outbound calls with members.
- Educate members about kidney health, related comorbid conditions, dialysis types, transplant options, and renal replacement therapy.
- Engage members in HealthMap's Kidney Health Program.
- Follow up with members according to complexity, cadence, and policy.
- Advocate for patients by responding to and working to resolve concerns or barriers.
- Use community resources and programs in care planning.
- Serve as a liaison among patients, their support networks, treating physicians, and ancillary providers to coordinate care, resolve nursing problems, and help patients meet individualized goals.
- Notify providers of identified patient needs according to policy.
- Comply with HIPAA privacy laws and applicable federal, state, and local regulations.
- Comply with company operational and security policies and procedures.
- Meet individual metrics and organizational key performance indicators.
- Navigate technical applications including Excel, OneNote, Outlook, and Word.
- Support after-hours coverage and various time zones based on business needs.
- Drive patients and families to participate in their own care and support self-management.
- Certified Case Manager certification is preferred.
- Three years of case management experience is preferred.
- Experience in a dialysis center or transplant center is preferred.
- Experience with Medicare and Medicaid is preferred.
- Strategic thinking and planning.
- Effective verbal and written communication.
- Ability to work in a challenging environment with changing priorities.
