Requirements
  • An associate degree and two or more years of experience, preferred, or four or more years of relevant experience.
  • CNA or CMT certification is preferred.
  • CPR or Basic Life Support certification is preferred.
  • Experience in a similar role, preferably within a senior living community or healthcare setting, is preferred.
  • Demonstrated proficiency with Microsoft Office programs including Outlook, Excel, and Word; web-based platforms; electronic health record systems; and applicable human resources systems.
  • Ability to walk or stand for extended periods and move throughout the community and resident apartments.
  • Ability to stoop, kneel, crouch, crawl, climb, and balance as required.
  • Ability to communicate effectively and identify and interpret written information.
  • Ability to lift up to 50 pounds independently and up to 200 pounds with assistance.
  • Ability to follow infection prevention and control protocols, including use of personal protective equipment when necessary.
  • Completion of required training and ongoing education initiatives.
  • Successful completion of a background check, drug screening, and required health assessments such as tuberculosis testing or physical evaluations.
Responsibilities
  • Develop and facilitate resident-directed programming under the direction of the Director of Nursing and Executive Director.
  • Assess residents’ needs upon move-in and incorporate activity goals into individualized service plans.
  • Coordinate and participate in resident orientation and identify potential changes or concerns in individualized service plans.
  • Work with staff, volunteers, residents, families, and extended communities to provide a psychosocial care model.
  • Provide leadership and education to care staff and families regarding individualized service plans and dementia-care best practices.
  • Mentor staff and volunteers, provide specific care training, and ensure resident care programs are implemented.
  • Coordinate care staff schedules, provide hands-on care as needed, and ensure adequate staff coverage.
  • Organize and conduct care staff orientation, mentorship, and ongoing training.
  • Ensure excellent resident care across all care levels in the community.
  • Participate in writing monthly calendars and newsletters as requested.
  • Encourage, assist with, and participate in resident activities.
  • Participate in interviewing and onboarding care staff.
  • Provide input on care staff performance and coordinate with the Director of Nursing on performance reviews.
  • Implement new-hire orientation and training for care staff.
  • Assess and complete nursing day-to-day duties during scheduled floor-nurse shifts.
  • Promote each resident’s physical, personal, and emotional well-being and support the highest possible level of self-care and independence.
  • Understand residents’ advance directives and follow their wishes.
  • Ensure effective communication with residents’ families regarding residents’ physical, personal, and emotional well-being.
  • Address conflict intervention and resolution for residents, families, employees, and others as needed.
