Requirements
  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.
  • Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age.
  • Final candidates are subject to successful completion of a background check. A drug screen or physical may be required during the post offer process.
Responsibilities
  • Participates in the transplant clinic; medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; medication reconciliation; coordination of patients' pharmaceutical needs with physicians, mid-level providers, and nurses.
  • Approves new drug orders and screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food interactions; participates in the pre-, peri-, and post-transplant management of patients in the hospital and in the transplant clinic.
  • Supervises Pharm.D. students, technicians, undergraduate and graduate student teaching, and provides educational programs for patients and hospital staff (physicians, nurses and pharmacy staff).
  • Participates in appropriate hospital and/or departmental committees relevant to transplant.
