Requirements
  • A qualified New York State driver's license is required and must be maintained throughout employment.
  • The ability to communicate and understand written and oral directives and information is required.
  • AMAP training is required and must be completed within six months of hire.
  • Successful completion of a fingerprint and criminal background check is required due to regular and substantial unsupervised or unrestricted physical contact with people receiving services.
Responsibilities
  • Ensure a safe and clean home by supporting people with household chores and participating in those chores as needed.
  • Follow all treatment plans and use the corresponding adaptive and safety equipment for each person to promote independence and ensure health, safety, and well-being.
  • Support people in preparing meals by teaching diet adherence, portion control, and cooking skills.
  • Assist people with medication administration according to AMAP policies and procedures.
  • Work with residential nurses to ensure accurate and current medical documentation for people supported, when maintained.
  • Implement and document treatment plans and agency materials, including residential habilitation goals, medical case notes, and communication logs, to support people in achieving independence and valued outcomes and to ensure their health, safety, and well-being.
  • Follow overtime policies and procedures to ensure quality support is provided each day.
  • Attend necessary meetings to discuss each person's progress and goal achievement and to exchange information and ideas.
  • Provide transportation as needed for emergency situations.
  • Mentor new Direct Support Staff by demonstrating supportive routines and reviewing training manuals to ensure new employees are prepared to complete their responsibilities.
  • Document overnight sleep checks for the people supported.
  • Know and participate in all emergency evacuation procedures.
  • Perform other job-related duties as requested by the supervisor.
Desired Qualifications
  • A high school diploma or equivalent is preferred.
  • First Aid, cardiopulmonary resuscitation, and SCIP-R training are preferred.
